schor, I'm sure you considered this but I was wondering if there might have been something in the pockets of the inserts that broke.
As you probably know the seat for the inserts must be dead flat for the insert.

Once I broke the first one I did check for any debris on all the next ones, but I did not see any and still broke some. I wonder if they were already slightly cracked.
