It looks like my Boyer Shultz 612. Right down to the table handwheel, the stop in the center, the little knob you see on the right side under the table and the writing above the grinding wheel (actually part of the casting) !st line: BOYER-SHULTZ, 2nd line: CORP, 3rd line CHICAGO, ILL. The difference is mine has the switch on the side, a lever.
