The star shaped objects are the cutters. The cylindrical parts align the keyway cutter in the bore and also are a 90 degree gearbox supplying power to the cutters.
Now what's really bizarre is a square hole cutter. The only one I've ever seen was on the USS Samuel Gompers and was used on the radial arm drill press. It consisted of a gear box that had a MT in order to mount it in the spindle and a really weird three flute cutter. Very similar to this one: http://gizmodo.com/5849810/how-the-hell-...hine-works
