It's largely superfluous to actually using the insert. In theory, you start with the fresh insert and the dot at the working corner. As the corner wears you index clockwise or counterclockwise, as you prefer and always the same.
In practice, I've never paid much attention to such things. A corner is a corner and I index in any direction as long as the worn edge isn't the one cutting anymore. The dot being there strikes me as being another case of a solution in search of a problem.
