I've always used a welding torch tip drill (AKA: a pin vice) and a tiny drill bit (cleaned with alcohol) that is turned with the good fingers on the other hand.
Light pressure, go slow, and as soon as it breaks through the nail... spurt!
Willie, There would be some protection form infection but it is more likely the fact that paper clips are cheap, readily available and disposable.
However, in Ed's case there would be no need to actually do any more than apply one of these:
