I've checked with Mitsubishi, NTK and Kyocera, nothing. I'm not saying someone doesn't have it, but if those three who specialize in small screw-down insert holders don't have it, you aren't going to have an easy time of it.
What's the maker and designation of the one you do have?

Thanks for checking Ken. The one I have is a Micro 100. They only make them for the smaller insert size though.

I did check the Toolmex site and they do them for negative inserts only. I knew I had seen them there.
